This is a Samaritan alphabet abecedary from a medieval Jewish manuscript called
Sha'ar Ha-razim, written in the late 1460s. The reading in the margin (in Arabic and Hebrew) is
כט אלסאמרה והו כתב עברי, 'the Samaritan alphabet, that is Old Hebrew.'
